Liability Waiver & Photo Release Form Liability Waiver & Photo Release I hereby release, indemnify, and hold harmless Friends of Georgia State Parks & Historic Sites, Inc. (“Friends”) and its agents or employees and all persons conducting or managing, directly or indirectly, the activities surrounding my involvement as a volunteer from any and all claims, rights, demands, actions, causes of action, expenses and damages arising from any injury, act or omission relating in the way to my participation as a volunteer. I understand that there are risks of injury or death or damage to property involved in my participation in such an event, that it is my responsibility to insure the safety of the equipment used and to see that it is operated properly and that Friends and its agents or employees assume no responsibility for the condition of such equipment, its operations or safety of the activity involved in this event. I understand that I am receiving no payments for services from Friends of Georgia State Parks & Historic Sites, Inc. I am not an employee. I am not entitled to and will not receive Worker’s Compensations benefits or other similar payments from Friends if I am injured. I agree to pay, protect, indemnify and save Friends harmless from and against all liabilities, damages, costs, expenses, causes of action, suits, demands, judgments, and claims of any nature whatsoever, including but not limited to any liability Friends may incur arising from, by reason of, or in connection with my participation of this event. I grant Friends and its agents or employees to use photographs and/or video and audio taken of me. These images may be used in educational and documentary materials such as Public Service Announcements, Grant Applications, Video Documentaries and both printed and online newsletters. Furthermore, I authorize the use of my image, likeness and voice for all program promotion, materials and any other purposes in connection with the program deemed appropriate and necessary by Friends including but not limited to advertising and marketing materials. I hereby agree to release, defend, and hold harmless Friends and its agents and employees, including any firm publishing and/ or distributing the finished product in whole or in part, whether on paper, via electronic media, or on the use of the photographs/video, including but not limited to any misuse, distortion, blurring, alteration, optical illusion, or use in composite form, either intentionally or otherwise that may occur or be produced in taking processing, reduction, or production of the finished product, its publication or distribution. I also waive any right I may have to inspect and/or approve the finished product of advertising copy that may be used in connection with my volunteer services. I hereby also agree to the above for my minor child(ren). I waive all claims for compensation for damages. I have read the release before signing below, fully understanding the contents, meaning, and impact of this release.
